How would you know that they're worn out, or need replacing?

If you hear clicking or ticking when you turn then its possible that its your joints. Also see if any of the boots are torn and leaking grease. Also check if the boots are still flexible and not tough or hard as they may need replacing.

what if it clicks while driving straight?

Okay, mechanic told me it was my cv joint. I begged to differ. Went around to all my bolts and stuff. Turned out he didn't tighten my lug nights fully when he did my clutch.

If it clicks, like a metal on metal noise when you are driving and attempting a turn, if its light you may be able to get away just with the boot if its the problem because when they rip, it looses grease which makes its operation rough and shatters the joints
